Case Manager RNs are responsible for the following:

  • Advocates for the patient and family throughout the entire episode of care.
  • Monitors patientrsquo;s progression intervening as needed to ensure efficient patient progress.
  • Coordinates all aspects of discharge planning and establishes a safe discharge plan.

The Case Manager RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population in order to facilitate patients through the continuum of care. Qualifications for this position include:

  • Bachelors of Nursing required
  • Three (3) years case management/discharge planning experience
  • CCM certification preferred
  • Active RN license in NJ/PA
